Beef and Barley Stew

The cheap cut and the forgotten grain, given three hours to become the best thing you eat all week. Barely any work, mostly waiting.

What this is

Beef, barley and root vegetables, cooked slowly until the meat gives way and the barley has thickened the whole pot.

It is the cheapest genuinely good meal I know how to make, it improves for three days, and the cut of beef you want is the one nobody else is buying.

Barley is also the ingredient on this list with the most serious evidence behind it, and the numbers are worth seeing.

Ingredients

For 6 servings

800 g beef shin, chuck or brisket, cut into 4 cm chunks 200 g pearl barley, rinsed 2 onions, diced 3 carrots, chunked 3 sticks celery, chunked 4 cloves garlic, minced 2 tbsp tomato puree 1.5 litres beef stock 2 tbsp olive oil 2 bay leaves 2 sprigs thyme 1 tbsp Worcestershire sauce Salt and black pepper

Optional: 200 ml red wine, 200 g mushrooms, a handful of parsley to finish

Method

1. Pat the beef dry and season it well. Heat the oil in a heavy pot and brown the meat in batches, about 3 minutes a side. Do not crowd the pot; grey meat means no flavour.

2. Remove the beef. Add the onion, carrot and celery to the same pot and cook 8 minutes, scraping up the browned bits from the base.

3. Add the garlic and tomato puree, cook 1 minute.

4. Add the wine if using and let it reduce by half.

5. Return the beef, add the stock, bay, thyme and Worcestershire sauce. Bring to a simmer.

6. Cover and cook on the lowest heat for 1 hour and 30 minutes, or in a 150C oven.

7. Add the barley and cook a further 45 minutes uncovered, until the barley is tender and the liquid has thickened. Add water if it gets too dry.

8. Season at the end, remove the bay leaves, and finish with parsley.

The carbohydrate quality evidence

This is the largest and best-designed study cited anywhere in this recipe collection, so it is worth going through properly.

Researchers set out to compare carbohydrate quality metrics against type 2 diabetes risk, because numerous such metrics have been proposed and the optimal one remained unknown. They systematically compared 23 different metrics, identified the five strongest associations, and proposed and tested an alternative index.

The data came from three prospective cohort studies: the Nurses' Health Study I from 1984 to 2020, Nurses' Health Study II from 1991 to 2019, and the Health Professionals Follow-up Study from 1986 to 2020. Participants were free of cancer, diabetes and cardiovascular disease at baseline.

During 5,628,955 person-years of follow-up among 213,704 adults, 22,351 cases of incident type 2 diabetes were ascertained.

Five and a half million person-years. Twenty-two thousand cases. This is what a serious nutrition evidence base looks like.

In multivariable-adjusted models comparing the highest fifth of intake to the lowest, cereal fibre carried a relative risk of 0.77, with a confidence interval from 0.74 to 0.81. Whole-fruit carbohydrates came in at 0.80, interval 0.76 to 0.84. Glycemic index ran the other way, at a relative risk of 1.20, interval 1.14 to 1.26.

Read those together. Higher cereal fibre intake tracked with 23 percent lower relative risk. Higher glycemic index tracked with 20 percent higher. Both intervals are tight, which is what a sample this size buys you.

Barley is a cereal fibre source. That is why it is in this pot rather than potatoes alone.

This is observational cohort data, so it establishes association rather than causation, and the cohorts are largely health professionals in the United States, which is not a representative population. Nothing here is medical advice about diabetes risk, which belongs with a clinician.

What barley brings specifically

A review sets out what is in the grain.

It examines the functional components of hull-less barley, noting the grain contains dietary fibre, protein, starch and malt among its key components.

The listed health associations are broad: cardiovascular protection, glycemic regulation, lowering blood cholesterol levels, reducing the risk of colorectal cancer, improving cellular signalling, producing short-chain fatty acids, and promoting the growth of beneficial gut microflora.

That is a long list, and long lists in review papers deserve scepticism rather than enthusiasm. These are associations and mechanisms described across a literature, not effects demonstrated by the review itself.

The short-chain fatty acid production is the one with the clearest mechanism: fermentable fibre reaching the colon is metabolised by gut bacteria into compounds that are themselves biologically active.

One piece of context the review offers that I found striking: hull-less barley accounts for only about 2 percent of total production for human food, with roughly 6 percent going to malt. Most barley grown does not end up on anybody's plate.

This is a narrative review covering composition, health associations and genetics, not a trial. Every health benefit listed is described rather than demonstrated by this source.

The fibre gap

One more piece of context that explains why any of this matters.

An analysis summarising a decade of research on whole grain and high-fibre cereals, cereal fibres and selected fibre sources examined digestive function, gut microbial effects, satiety signalling, body weight management, cardiovascular disease and blood glucose control.

Its framing is the useful part: dietary fibre intake remains significantly lower than recommended levels, particularly in North America, and the review calls this fibre gap one of the most important issues facing public health nutrition.

So the reason to put barley in a stew is not that it is a superfood. It is that most people are eating far less fibre than recommended, and a grain that thickens a stew while carrying fibre is an easy way to close some of that gap without changing how you eat.

The review also notes that fibre amount, fibre type, processing techniques and the associated nutrients and phytochemicals are all critical factors affecting outcomes, which is a caution against treating all fibre as interchangeable.

Storage and swaps

Keeps 4 days in the fridge and is better on day two and three, which is unusual and genuinely true here. Freezes for 3 months.

It thickens dramatically as it cools because of the barley. Loosen with stock or water when reheating.

Use the cheap cut. Shin and chuck have the connective tissue that turns to gelatin over a long cook, which is what gives the stew its body. An expensive steak cooked this way is a waste of money and comes out worse.

Swap barley for farro or spelt, both of which behave similarly. Rice does not; it collapses.

Add mushrooms with the onions for depth, and add them generously.

And give it the full time. This is not a dish you can rush; ninety minutes before the barley goes in is the minimum.

The takeaway
