How To Do the Leg Press

The leg press gets dismissed as the lazy squat. An MRI study of seventeen separate muscles suggests it earns its place, with one clear gap.

The leg press is a machine where you push a weighted platform away with your feet while your back is supported.

It lets you load your legs heavily without balancing a bar, without your lower back being the limit, and without much technical skill.

That is genuinely useful. But there is one specific thing it does not do, and a study measured exactly how much it does not do it.

The MRI study that should change your leg day

Seventeen untrained adults trained one leg with knee extensions and the other leg with leg presses. Same person, both conditions, which removes the usual noise between people.

The protocol was identical for both: 70 percent of one-rep max, 10 reps per set, 5 sets per session, twice a week, for 12 weeks. Afterwards, MRI measured the volume of 17 individual muscles, including all four heads of the quadriceps, the glutes, hamstrings and adductors.

Both exercises significantly grew the quadriceps overall. With one exception.

The rectus femoris did not significantly grow in the leg press condition, p equals 0.379.

And the size difference is not subtle. Rectus femoris gains were 13.2 percent for knee extension against 1.1 percent for leg press.

The vasti, the other three quad heads, grew similarly in both.

Here is why that happens, and it makes sense once you see it. The rectus femoris is the only quad head that crosses the hip as well as the knee. On a leg press your hip and knee both bend and straighten together, so the rectus femoris is shortening at one end while lengthening at the other and never gets properly loaded. A knee extension keeps the hip still and makes it do the work.

So the leg press builds three of your four quad heads well and essentially skips the fourth.

What to do about it

Do not throw out the leg press. It grew the vasti as well as knee extensions did, and it lets you load heavy with a supported back.

Just do not assume it covers everything. If leg press is your only quad exercise, add some knee extensions, or something that loads the knee with your hip held relatively straight.

That is the practical version of a 13.2 versus 1.1 percent gap.

Before you over-commit: 17 untrained adults over 12 weeks. Untrained people grow from almost anything, and whether the same gap appears in trained lifters over longer periods is not established here.

Two legs are not twice one leg

Thirty-one resistance-trained men were tested for one-rep max leg press in three loading modes: normal bilateral, unilateral, and split-load where the two legs are loaded differently. They were also tested for isokinetic knee flexion, knee extension and hip extension strength.

The split-load condition produced a significantly greater bilateral deficit ratio, 14.29 percent plus or minus 7.71, against 5.16 percent plus or minus 9.60 for the normal bilateral condition.

The bilateral deficit is the well-known observation that your two legs working together produce less than the sum of each working alone.

The authors' practical conclusion: the split-load leg press is a useful way to test for that deficit because the numbers are easy to detect, and the unilateral leg press is useful for spotting side-to-side strength differences because it related to isokinetic knee strength.

So if you want to find out whether one leg is quietly carrying the other, press one leg at a time. The machine is good at revealing that, precisely because it removes balance from the equation.

Why depth is doing more than you think

A review examined how muscle length, range of motion and exercise selection interact for muscle growth.

Its synthesis across several lines of evidence, including isometric training at different muscle lengths, training with different ranges of motion, and exercises that load muscles at different lengths, is that muscle length appears to play a real role, with training at longer muscle lengths producing more favourable growth.

The authors are honest about the difficulty: it is hard to isolate whether muscle length itself is the driver, because range of motion and exercise selection are tangled together with it.

For the leg press that means the bottom of the rep, where your quads are most stretched, is likely the valuable part. Cutting the range short to load more plates is trading away the portion that may matter most.

This is an unstructured review, and the authors flag the difficulty of drawing firm practical conclusions. Treat it as a lean, not a law.

Depth and foot position, honestly

You will find strong opinions about exactly where to place your feet. I did not find studies in this search that settle it.

What I can reason from the evidence above: the muscles that grew on the leg press were the vasti, and the mechanism behind the rectus femoris result is about hip and knee moving together. Foot position changes leverage but does not change that basic relationship.

The depth guidance is simpler. Go as deep as you can without your lower back peeling off the pad, because that is where the machine stops protecting your spine and starts loading it in a rounded position. Keep your whole foot in contact with the platform.

I am labelling the foot position part as reasoning, not a finding.

How to actually do it

Set the seat so that at the bottom your knees bend deeply but your lower back stays flat against the pad.

Feet about hip to shoulder width, flat on the platform, whole foot in contact.

Lower under control. The machine will let you drop the weight fast because it is guiding the path. That is not an invitation.

Stop lowering the moment your pelvis starts to tuck and your lower back lifts off the pad. That is the actual depth limit, and it is different for everyone.

Press back without locking your knees out hard at the top.

Use one leg at a time occasionally. It is the cleanest way to find out if you have a weaker side.

End the set when your lower back starts lifting off the pad at the bottom.

The takeaway

The leg press builds three of your four quadriceps heads well, and essentially skips the rectus femoris: 1.1 percent growth against 13.2 percent from knee extensions in a 12-week MRI study. If the leg press is your only quad work, add knee extensions to cover the gap. Keep your lower back flat against the pad and let that, rather than a target depth, decide how low you go. And press one leg at a time occasionally, because the machine is unusually good at exposing a weaker side once balance is taken out of the equation.

Limits of this evidence

  • The hypertrophy study used 17 untrained adults, where growth occurs readily from most stimuli, so whether the rectus femoris gap persists in trained lifters is not established.
  • That study ran 12 weeks at a fixed protocol (70 percent 1RM, 5 sets of 10, twice weekly). Different loads, ranges or frequencies were not tested.
  • It used a within-person design with each leg training a different exercise, which controls between-person variation but cannot fully rule out cross-education between limbs.
  • The bilateral deficit study used 31 trained men only, so it does not speak to women or untrained people.
  • No study here tested foot placement or depth on the leg press, so that guidance is mechanical reasoning rather than a measured result.
  • No source measured injuries, so the advice about the lower back lifting off the pad is a position argument, not injury evidence.
  • Each claim rests on a single source with no independent replication checked. Treat the piece as provisional.
