How To Do the Push-Up
Hands, hips, and shoulder blades. Here's what's actually worth caring about, plus one hand trick that might change what your shoulders do.
The straight answer
Set your hands, lock your body into one line, and lower with control. That's the whole game.
Here's the shape. Hands about shoulder-width. Body straight from your head to your heels, so your hips don't sag and your butt doesn't spike. Lower yourself slow. Push back up.
Why the straight-line body? Because a push-up is a full-body move pretending to be an arm move. Your midsection has to stay tight so the force can travel through you cleanly. When your middle holds firm, your shoulders, arms, and legs all work off the same solid base. Think of your body like a plank of wood laid across two chairs. Push down on a solid plank and it holds. Push down on a wet noodle and it folds in the middle. Your job is to be the plank.
Where that picture breaks: a plank is dead weight, and you're not. You're actively bracing, which means you can lose the brace the second you stop paying attention. A plank never gets tired. You will.
Set the base first
Before you drop, get tight everywhere.
Squeeze your glutes. Brace your stomach like you're about to take a light punch. Now your hips can't sag and your low back can't cave. That tight middle is what keeps the whole line honest.
Why I keep hammering this: a firm middle is what lets you hold posture, stay balanced, and pass force from the floor through your body. That's the reason a good push-up feels like your whole body is working, not just your chest. Take that as solid thinking, not a proven law. It comes from background reasoning, not a hard test.
Control the way down
Don't drop. Lower.
Slow the way down and you keep control of the force going through your hands and shoulders. Speed changes how hard the move hits. When people tested push-ups at different speeds, the pace you moved at lined up with the peak force pressing into the ground. Slower and steadier gave the cleanest, most predictable numbers.
Also true from the same testing: heavier people pushed more force into the floor. So a bigger guy and a smaller guy doing the same push-up are not doing the same amount of work. That's not a flaw in you. It's just physics.
Both of those come from one small study of 27 young men. Treat them as a maybe that points the right way, not as settled fact.
The hand trick worth knowing
Here's the part most people never hear. Where you point your hands changes what your shoulder blades do.
When guys did push-ups with their hands rotated to different angles, their shoulder blades moved differently for each angle. Turn the hands out, and the shoulder-blade motion shifted one way. Turn them in or keep them neutral, and it shifted another way. So your hand position isn't just comfort. It's steering your shoulder blades.
One more layer. Guys who already had long-term shoulder pain didn't respond the same way as pain-free guys. Same hand positions, different shoulder-blade behavior. So if your shoulder already bugs you, don't assume the pain-free crowd's setup fits you.
All of this is from one study on male overhead athletes. It's a real signal, but it's thin. Read it as 'interesting, worth trying,' not 'law of the gym.'
Why the shoulder cares so much
Your shoulder likes this kind of pushing.
A push-up is a closed-chain move, meaning your hands are planted and your body moves. One line of thinking says the shoulder is built well for stability and force when your hands are fixed like that. The idea goes further: a lot of modern training leans on hands-free, heavy pulling and pressing, which might load the shoulder in a rougher way and quiet down the muscles that stabilize your shoulder blade. Push-ups may lean the other direction, keeping those stabilizers in the game.
I want to be honest with you here. This is a hypothesis. Somebody proposed it in a review, and it sounds reasonable, but it hasn't been nailed down. Shoulder problems come from a pile of causes: age, how your shoulder's built, overload, and general health stuff. No single move is a magic shield. So do push-ups because they're a good pattern, not because you think they'll bulletproof your shoulder.
Making it harder the smart way
Once the standard push-up feels solid, you can turn up the shoulder-blade work.
A push-up-plus is where you finish the push-up, then push a little more so your shoulder blades spread apart at the top. When guys added a single-leg version of this, the serratus anterior (a key shoulder-blade muscle) lit up more. Reaching through a bigger shoulder-blade range also pulled in more of the trapezius. And the balance between those muscles looked more like a stable, healthy pattern.
That's from small studies on trained or Pilates-experienced men. Good direction, thin proof. But if you want more from the same movement, this is a clean way to earn it.
Set your hands about shoulder-width. Brace your stomach and squeeze your glutes so your body is one straight line. Lower slow, push back up, keep the line the whole way. If your shoulder feels off, play with pointing your hands slightly differently, since hand angle changes what your shoulder blades do. And when the basic version gets easy, add the push-up-plus to pull more shoulder-blade muscle into the work. None of this is bulletproof science, but the plain moves here are worth trusting.
Limits of this evidence
- The core-stability point is background reasoning, not a tested result, and that source rates its own evidence as thin.
- The force and tempo findings come from one small study of 27 young male students, so they may not carry over to everyone.
- The hand-position and shoulder-blade findings rest on a single study of male overhead athletes, with no second source to check it against.
- Pain-free guys and guys with long-term shoulder pain reacted differently to the same hand positions, so one setup won't fit everyone.
- The idea that push-up-style loading protects the shoulder is a proposed hypothesis, not something that's been tested and confirmed.
- Shoulder problems have many causes (age, build, overload, general health), so no single exercise is a guaranteed shield.
- The push-up-plus muscle findings come from small, narrow samples of trained or Pilates-experienced men, so treat them as a maybe.
- Across the board, most of this leans on single studies, so I'm keeping the confidence at solid-but-not-proven at best.
