Does Apple Cider Vinegar Work?

Apple cider vinegar is the rare wellness product with real randomised trials behind it. Then you look at how much weight people actually lost.

The claim

A shot of apple cider vinegar before meals burns fat, controls blood sugar and generally sorts you out.

This one is genuinely interesting, because unlike most of this category it has been tested repeatedly, pooled, and then pooled again at a higher level.

So I can give you actual numbers rather than a shrug.

The top of the evidence pyramid

An umbrella review is a review of meta-analyses. It sits above individual trials and above individual pooled analyses, and it is about as high as this kind of evidence goes.

Researchers searched four databases up to January 2025 and included 10 meta-analyses covering 38 primary studies, re-estimating effect sizes with a random-effects model.

Here is what vinegar consumption was associated with:

Fasting blood glucose down 9.40 mg/dL. Postprandial glucose down 14.59 mg/dL. Postprandial insulin down 1.29 mu/L. HbA1c down 0.70 percent. Total cholesterol down 9.39 mg/dL. Body weight down 1.06 kg. Systolic blood pressure down 2.94 mmHg.

Those are real, statistically pooled effects across 38 studies. Vinegar is not doing nothing.

Now read the weight number again. 1.06 kg. Just over a kilogram, in studies where people were taking it consistently.

The glucose effects are the more interesting half. An HbA1c reduction of 0.70 percent is not trivial in a metabolic context, and that is where vinegar's genuine story lives, not in fat loss.

The weight loss meta-analysis, specifically

A separate systematic review and meta-analysis focused on apple cider vinegar and body composition. It searched four databases up to March 2025 for randomised trials of at least four weeks in adults, with risk of bias assessed using the revised Cochrane tool.

From 2,961 reports screened, 10 randomised controlled trials with 789 participants qualified.

Daily apple cider vinegar significantly reduced body weight, standardised mean difference minus 0.39, and BMI, standardised mean difference minus 0.65.

The review's own framing before doing this work: its effects on weight loss remain controversial.

Two things to hold together. The effect is statistically real and replicated. The heterogeneity was 62 percent, which means the underlying studies disagreed considerably with each other.

And there is a population detail that matters enormously: this analysis was in humans with type 2 diabetes and/or overweight. People with metabolic dysfunction. That is a different starting point from a healthy 22-year-old wanting sharper abs.

Why the numbers are smaller than the marketing

Put the two analyses side by side and the picture is consistent.

Vinegar has a modest, genuine effect on glucose handling, and a small effect on weight, concentrated in people whose metabolism is already impaired.

About a kilogram, over weeks, in that population.

A kilogram is a real thing. It is not a transformation, and it is nowhere near what the before-and-after content implies. Nobody sells a supplement on the promise of one kilogram.

Also worth noticing: both analyses studied vinegar as a food, consumed with meals. Not gummies, not capsules. A gummy that removes the acetic acid and adds sugar is a different product than the one that was tested.

What acetic acid does at the cellular level

If you want the mechanism rather than the outcome, one study went looking in mice.

Forty female mice were assigned to five groups covering ovariectomy, exercise, acetic acid supplementation, and both interventions together, over a 13-week intervention using 5 percent sodium acetate chow and voluntary wheel running. Body composition, glucose tolerance, energy expenditure and skeletal muscle mitochondrial function were assessed.

Acetic acid is the active component of vinegar, so this is asking what the ingredient does inside muscle rather than what the drink does on a scale.

Ovariectomy impaired whole-body metabolism and mitochondrial function in the calf muscle, and notably, exercise alone failed to fix the mitochondrial dysfunction it caused.

The reason I include this is not to claim it applies to you. It plainly does not: mice, a surgical menopause model, and sodium acetate in chow rather than vinegar in water. It is here because it shows the metabolic story is where researchers actually look, which lines up with the glucose findings above being stronger than the weight findings.

The part that can actually hurt you

Vinegar is an acid, and it goes past your teeth and down your oesophagus.

Undiluted vinegar erodes tooth enamel, and enamel does not grow back. It can also irritate the throat and worsen reflux in people prone to it.

If you drink it, dilute it in water, drink it through a straw, and rinse your mouth with plain water afterwards. Do not brush immediately, because enamel is softened right after an acid exposure.

I should be straight that this dental and oesophageal point is established property of acids rather than a finding from the two analyses cited here, neither of which was primarily a safety study.

So is it worth it

If you have impaired glucose control, the evidence here is genuinely the strongest of anything in this category, and it is cheap. Worth a conversation with whoever manages your health.

If you are a healthy person chasing fat loss, the honest number is about a kilogram, in a population that was not you, alongside heterogeneous studies.

If you are buying gummies, you are buying a different product to the one that was tested.

And if you drink it neat, you are trading your enamel for a kilogram.

The takeaway

Apple cider vinegar is the rare wellness product with real pooled evidence, and the numbers are modest. Dilute it, use a straw, rinse afterwards, and know that gummies are not the thing that was tested.

Limits of this evidence
